Keeping people safe is at the heart of the Yorkshire & Humber Care Record (YHCR). Interweave, the technology behind YHCR, follows a clear and trusted Clinical Safety Assurance process based on NHS Digital standards DCB 0129 and DCB 0160.
How We Keep Data Use Safe
We have a dedicated Clinical Risk Management System (CRMS) and an accredited Clinical Safety Officer (CSO) who is a registered healthcare professional trained in clinical risk management. The CSO oversees safety assessments, reviews risks, and ensures all documentation meets national standards.
We work closely with partner organisations and clinical teams to identify potential risks early, put safeguards in place, and keep all safety information up to date.
What’s Expected of Partner Organisations
Any organisation connecting to YHCR needs to follow clinical safety requirements. This means appointing a Clinical Safety Officer or Clinical Lead and completing the required safety documents.
- Providers (Data Suppliers): Must check the quality and accuracy of data before it is shared, run user testing, and complete clinical verification before going live.
- Consumers (Data Users): Must carry out their own safety testing, confirm data displays correctly, and share testing evidence before go-live.
Shared Safety Information
We maintain central hazard logs for the Interweave platform so that identified risks and mitigation are visible to all partners. This helps organisations learn from each other and manage risks consistently.
Extra Help for Small Providers
We know smaller organisations may need more support with clinical safety. We offer template documents, guided workshops, and one-to-one support to help them meet the required standards and complete the process confidently.

Interweave connects a growing network of Clinical Safety Officers and Clinical Safety Leads across the region. This community shares best practice, advice, and support, helping us all deliver safer care through better information sharing.
