The national data opt-out was introduced on 25 May 2018, enabling patients to opt out from the use of their data for research or planning purposes, in line with the recommendations of the National Data Guardian in her Review of Data Security, Consent and Opt-Outs.

Local Health & Care Records
In Yorkshire & Humber there are a number of local health & care records that already exist. Yorkshire & Humber Care Record is working with these areas to help build on their knowledge and expertise to ensure all areas across the region benefit from their information being shared.
Leeds Care Record is a joined-up digital care record which enables clinical and care staff to view real-time health and care information across care providers and between different systems. It is a secure computer system that brings together certain important information about patients who have used services provided by their GP, at a local hospital, community healthcare, social services or mental health teams.
The Rotherham Health Record is an electronic system for sharing your health information in a secure way with health and care staff who provide care directly to you. This gives them access to the most up-to-date information so that they can provide better and quicker care.
Health and care professionals, including doctors and nurses, who are directly providing your care, will see a summary of your existing records – such as those held by your GP, hospital or social care provider – to allow them to make the right decisions with you and for you. You will only have to tell your story once.
